# Position Information

Job Description
Provides case management for at risk clients and their families primarily through the Baby Care program for maternity, infants and young children. Provides direct nursing services in the clinic, home, and office. Develops and implements plan of care based on client assessment. Independently assesses individual, family and community health and determines interventions needed, such as immunizations, dental varnish, infant car seat training, referrals to other agencies, etc. Decides priorities for client contacts (frequency and type). After orientation, carries a minimum caseload of 25 Baby Care clients. Does outreach to private providers, WIC, Department of Social Services, hospitals and other community groups/agencies to promote the mother/infant home visiting program.
Schedules workload efficiently and effectively within the time available. Daily makes decision on situations that arise unexpectedly, determining level of complexity, choosing appropriate intervention, and involving supervisory staff as needed. Maintains basic knowledge of current public health problems and issues in the community. Serves on community taskforces, committees, etc. to represent the Health Department.
Participates in community-planned education such as health fairs, community classes, school events and in-services as appropriate.
Maintains or enhances professional performance through district and local office staff in-service, workshops, seminars, web net, teleconference, polycom, professional journals, etc.
May be required to perform other duties as assigned. May be required to assist state government, generally, in the event of an emergency declaration by the Governor. May be required by VDH to adjust work hours, location and job duties and participate in training to meet public health business needs, including emergencies.
## Minimum Qualifications
Registered Nurse licensed in Virginia; clinical experience working with maternity, infant and young child health.

Public health/community health experience; bilingual in Spanish;
